Parental request for school to administer prescribed medication
Please note the school will not give your child medication unless it has been prescribed by the doctor and you have completed this form, or the paper version available from the school office. At the end of the school day, the medicine must be collected from the school office. If your child is going to Children’s Choice Childcare After School club, you must advise staff of this by calling Sabiha on 07516 697 365 to make arrangements for this medicine to be given to staff.
